You could also fax records to: 541-610-1746

Requesting Records

If you’re requesting records, please email Dr. Kremer at contact@chiro4critters.com

Make sure to include:

Your full name
Your pet’s name
Which dates of service you are wanting records for
The purpose for requesting these records

Please allow up to 14-days to receive the records.  If the nature of your request is for a veterinarian due to an emergency, please indicate this, so Dr. Kremer can get this to them in an expedited time-frame.
